Pediatric Primary Care Physician - Full Time - Hartford, CT

Responsibilities

• Provides primary medical care for a dedicated panel of patients, including diagnosis and treatment of acute and chronic illness according to evidence-based standards.
• Obtains clinical histories, conducts appropriate physical examinations and assessments, orders diagnostic testing and formulates treatment plans.
• Counsels patients and their families on preventive health measures and screening tests as appropriate.
• Orders and is responsible for the dispensing and administration of certain medications and immunizations on-site.
• Documents appropriate clinical information in the electronic health record in a timely manner and completes billing.
• Reviews documents including outside records and consultant reports and summarizes information in the medical record.
• Determines need for additional services including but not limited to nursing, care coordination, social services, health education, and referral for specialty care.
• Participates in regular multidisciplinary clinical team meetings and assists in the coordination of care in a team-based setting.
• Refers patients to hospitals for emergency care and admission when needed, and communicates with hospital staff as appropriate.
• Uses and promotes evidence-based clinical practice.
• Reviews on a regular basis long term cases that require ongoing medical attention.
• Consults with Medical Director and other medical professionals as needed regarding patient care, assessment, and education issues.
• Participates in quality improvement, management, continuing education, and other patient care programs established by the hospital.
• Participates in the identification and implementation of opportunities to enhance patient and family satisfaction.
• Participates in efforts to ensure compliance with JCAHO standards and other applicable accreditation and regulatory standards.
• Participates in teaching and learning in an integrated academic model.
• Participates in special programs and services as assigned.
• Assists in the resolution of complaints, requests and inquiries from patients.

Qualifications

• MD or DO Degree, successful completion of a pediatric residency program.
• Completion of a chief residency or a minimum of 1 year of practice experience preferred.
• Licenses and certifications: Board Certification/Board Eligible Pediatric Medicine, Registration certificate from the Drug Enforcement Administration ("DEA certificate").
