Core Responsibilities And Duties
- Completes an evaluation utilizing principles of family-centered care.
- Designs assessment consistent with goals of evaluation, provider order, patient / family input and priorities.
- Selects evaluation methods, tools, and environment to obtain valid and reliable assessment specific to the patient’s age and developmental, neurological, musculoskeletal, cardiopulmonary, cognitive and behavioral status.
- Administers evaluation consistent with best practices and Gillette competencies.
- Interprets evaluation for to the patient / family and seeks their assessment of the evaluation’s reliability and validity.
- Makes recommendations for treatment, referrals, equipment, resources and home and community based activities.
- Documents and bills for evaluation.
- Implements a therapy care plan utilizing principles of family-centered care.
- Designs and implements a therapy care plan including frequency, intensity, duration, and modalities.
- Collaborates with related services and home and community based resources and referrals and in the context of the patient and family’s needs, preferences and priorities.
- Identifies desired functional outcomes of the episode of care and goals for initial care plan recertification.
- Maintains progress toward goals and objectives, identifies need for modification of goals or intervention program and appropriate discontinuation of intervention.
- Periodically completes evaluation and assessment to determine if amount and rate of progress is appropriate.
- Designs home programs that facilitate goal attainment and generalization of new skills to home and community.
- Prepares the patient and family for discharge from therapy and indicates for follow-up therapy care.
- Completes necessary administrative and procedural tasks in a timely manner.
- Completes timely and complete documentation to assure effective communication of the plan to the ordering provider, the patient/family, and payers and to assure continuity of care (Same day documentation – 80%).
- Completes safety, incident, time-keeping, and customer service documentation same day.
- Uses time productively in direct and indirect patient care, department contributions and assisting team members as needed.
Qualifications
- Professional Licensure/Registration: Occupational Therapy State License.
- Pending credentials: Registration and Licensure pending, not to exceed three months in this status for OT.
- Current certification in CPR (BLS for Healthcare Providers through American Heart Association) or must obtain certification within 90-days of hire.
- Pediatric experience strongly preferred.
- Knowledge, Skills, Abilities: Competency in occupational therapy evaluation, treatment and related family/community education and planning for transition to community services; Knowledge of child growth and development, rehabilitation theory and practice with a wide variety of childhood onset diagnoses and related considerations including anatomy and neurology, emotional behavior factors, family needs and issues, community transition and equipment; Effective verbal and written communication; Ability to function as an effective team member; Excellent customer service skills.
