Full-Time On-site position in Springfield, MA 01103, USA. Reports to the Chief Medical Officer.
Requirements
- Current Massachusetts NP licensure and/or eligibility for certification
- Current Massachusetts CSR and DEA licenses
- Certification in Pediatrics or certification in Family Practice with a special interest in pediatric & adolescent medicine
- Ability to collaborate with a multidisciplinary team, including physicians, PAs and NPs, nursing staff, medical assistants, community health workers, nutritionists, social workers, and case managers
- Previous experience in ambulatory care and/or public health is preferred
Desired Qualifications
- Fluency in Spanish, Arabic, Russian, Nepali, Pashto, Dari, Swahili, or another language served at CHC
- Special expertise in provision of health care to underserved, culturally diverse populations
- Proficiency in assessment and delivery of family planning services to a diverse population
- Interest in developing and maintaining collaborations with outside institutions and agencies that play a critical role in a child and adolescent's health, such as schools and behavioral health agencies
Responsibilities
- Conducts comprehensive patient assessments—both in-person and via telemedicine (audio and video)
- Identifies potential and actual health and health-related problems, both acute and chronic, and develops and implements treatment plans with the patient and family/guardian, including pharmacologic and non-pharmacologic components
- Incorporates discussions on wellness, health promotion, disease prevention, and self-care into treatment plans
- Assesses social determinants of health that may impact a child’s or adolescent’s health trajectory
- Provides comprehensive clinical family planning services when appropriate
- Reviews patient charts to ensure health maintenance issues are addressed in a timely manner and appropriate treatment plans are developed and followed
- Completes all charts within 72 hours of the visit, adhering to risk management and chart audit requirements
- Collaborates and consults with CHC physicians and other providers, especially for complex patients
- Refers patients to appropriate specialists and other practitioners or facilities as needed
- Participates in the Pediatric "On Call" schedule
- Cooperates with CHC administration to monitor and improve the quality of patient care by evaluating patient outcomes
- Maintains updated Pediatric Nurse Practitioner guidelines and those developed by the American Academy of Pediatrics
- Participates in peer review of charts
- Maintains all CME/CEU, Board, and licensing requirements of state and national governing boards
- Complies with Caring Health Center’s compliance standards, including its Standard of Conduct, Compliance program, and policies and procedures
- Ability to use a computer, work at a computer workstation, view a CRT, and have basic keyboard knowledge
- Ability to work long and arduous hours as needed
Pay
Salary ranges from \$125,000 - \$167,990.
