Role Overview
The Pediatric Case Manager supports patients and families throughout their care journey by coordinating services, developing individualized care plans, and facilitating communication among care teams.
Key Responsibilities
- Guide patients and families through program orientation
- Explain rehabilitation and insurance benefits
- Coordinate education
- Complete psychosocial assessments
- Ensure seamless, patient-centered care in collaboration with interdisciplinary teams
Qualifications
- RN license required
- Bachelor's degree preferred
- Minimum of 2+ years in Case Management or 3+ years in clinical nursing
- InterQual experience preferred
