Pediatric Nurse Case Manager

Role Overview: The Pediatric Case Manager supports patients and families throughout their care journey by coordinating services, developing individualized care plans, and facilitating communication among care teams. Key Responsibilities: Guide patients and families through program orientation, explain rehabilitation and insurance benefits, coordinate education, complete psychosocial assessments, and ensure seamless, patient-centered care in collaboration with interdisciplinary teams. Qualifications: RN license required; Bachelor's degree preferred; minimum of 2+ years in Case Management or 3+ years in clinical nursing; InterQual experience preferred.
