Pediatric Home Health LVN

Role: Pediatric LVN providing one-on-one skilled nursing care in patients' homes. Responsibilities Administer medications and deliver treatments to medically fragile children.Monitor patient conditions, including trach, vent, and G-tube care as needed.Document care in EMRs and follow individualized care plans.Collaborate effectively with families and the clinical team. Qualifications & Skills Licensed Vocational Nurse (LVN) with pediatric care experience preferred.Strong clinical skills in medication administration and patient monitoring.Excellent communication and teamwork abilities.Ability to work independently in a home-care setting.
