*Pediatric Float RN: Hospital to Home w/Sign-On Bonus, Overnight Shift

About the role

Join Pediatric Home Service’s new Hospital‑to‑Home team as a Pediatric Hospital‑to‑Home Nurse. You will act as a bridge between the hospital and families, educating clinicians, building trust, and ensuring medically complex children transition safely back to their homes. The position is based in the Minneapolis‑St. Paul Metro Area with travel throughout Minnesota as needed.

Responsibilities

• Educate hospital clinicians and families about home care requirements for medically complex children.
• Build trust with families and support them during the transition from hospital to home.
• Provide direct, one‑on‑one nursing care in the home setting, including management of tracheostomies, ventilators, and G‑tubes.
• Float between hospital and home environments, caring for patients of varying ages and medical technologies.
• Collaborate with the interdisciplinary team to develop and implement individualized care plans.

Qualifications

• Active RN license.
• Minimum of 3 years of pediatric nursing experience, including care for medically complex children.
• Willingness to work overnight and weekend shifts.

Pay

Hourly rate for RN candidates ranges from $45.00 to $50.00, based on license, experience, qualifications, scheduled hours, and client acuity.
