Responsibilities

  • Delivering and coordinating quality patient care while observing and assessing patient conditions
  • Documenting assessments, nursing actions, and responses accurately and timely
  • Participating in and updating nursing care plans to achieve optimal outcomes
  • Recognizing and responding to changes in patient needs to prevent or reduce risks
  • Educating patients and families on disease management, self-care, and prevention strategies
  • Maintaining infection control standards and complying with policies, regulations, and the Nurse Practice Act

Qualifications

  • An active, unrestricted LPN/LVN license in the practicing state
  • Current CPR certification
  • Recent clinical assessment and documentation proficiency
  • Reliable transportation and the ability to travel as needed
  • Additional state-specific experience requirements and continuing education may apply

Skills

  • Attention to detail, time management, problem-solving, and strong communication skills
  • Physical ability to lift up to 50 pounds, reposition patients, and perform various physical tasks
  • Ability to work in diverse environments, including exposure to infectious materials
