Responsibilities
- Delivering and coordinating quality patient care while observing and assessing patient conditions
- Documenting assessments, nursing actions, and responses accurately and timely
- Participating in and updating nursing care plans to achieve optimal outcomes
- Recognizing and responding to changes in patient needs to prevent or reduce risks
- Educating patients and families on disease management, self-care, and prevention strategies
- Maintaining infection control standards and complying with policies, regulations, and the Nurse Practice Act
Qualifications
- An active, unrestricted LPN/LVN license in the practicing state
- Current CPR certification
- Recent clinical assessment and documentation proficiency
- Reliable transportation and the ability to travel as needed
- Additional state-specific experience requirements and continuing education may apply
Skills
- Attention to detail, time management, problem-solving, and strong communication skills
- Physical ability to lift up to 50 pounds, reposition patients, and perform various physical tasks
- Ability to work in diverse environments, including exposure to infectious materials
