About the Role

The primary purpose of this position is to provide primary care to specific residents under the medical direction and supervision of the residents’ attending physicians or the Medical Director of the facility. You will focus on evaluation, observation, data collection, illness prevention, and health care management. Additionally, you will assist in modifying the treatment regimen to meet the physical and psychosocial needs of residents, in accordance with established medical practices, state requirements, and facility policies.

Responsibilities

  • Administrative Functions
    • Confer with the Medical Director and attending physicians regarding assigned residents.
    • Consult with physicians concerning resident evaluation and assist in planning nursing services.
    • Initiate requests for consultation or referral and respond to requests from residents, physicians, or nursing staff.
    • Examine residents and their records to distinguish between normal and abnormal findings, recognizing early signs of physical, emotional, or mental problems.
    • Determine when to refer residents to a physician for evaluation or direction.
    • Complete medical forms, reports, evaluations, studies, and charting for assigned residents.
    • Implement and maintain facility policies, procedures, objectives, quality assurance, safety, and infection control standards.
    • Provide written and oral reports to physicians, the Medical Director, or Administrator regarding resident status and care.
    • Participate in planning and evaluating nursing service policies and procedures.
    • Assist in facility surveys conducted by government agencies and support the Quality Assessment and Assurance Committee in addressing identified deficiencies.
  • Committee Functions
    • Serve on the Interdisciplinary Care Plan Team to develop comprehensive resident assessments and care plans.
    • Provide reports on resident status as requested by committees.
    • Evaluate and implement committee recommendations related to medical activities and resident care.
    • Participate in facility committees (e.g., Infection Control, Policy Advisory, Pharmaceutical, Budget, Quality Assessment and Assurance).
    • Schedule, attend, and contribute to Care Plan committee meetings and Discharge Plan reviews.
  • Personnel Functions
    • Cooperate with personnel to achieve department objectives and maintain good employee relations.
    • Meet with the Medical Director, physicians, Director of Nursing Services, and nursing staff to discuss resident care.
    • Maintain effective working relationships with health professionals, consultants, and family members.
    • Conduct performance evaluations in accordance with facility policies.
    • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
  • Medical Care Functions
    • Welcome and orient residents to the facility environment.
    • Provide information to residents and families about resident rights.
    • Perform evaluations, observations, data collection, and diagnostic services for assigned residents.
    • Conduct in-depth physical examinations using techniques such as observation, inspection, auscultation, percussion, and palpation.
    • Record physical findings and formulate care plans and prognoses; report to attending physicians.
    • Order, interpret, and evaluate diagnostic tests to assess resident needs and report findings to physicians.
    • Provide therapeutic services and collaborate with the Care Planning Team to develop care plans.
    • Manage minor illnesses (e.g., colds, diarrhea, skin abrasions, sprains) and uncomplicated conditions (e.g., diabetes, hypertension) under established protocols.
    • Monitor and adjust medical regimens for chronically ill residents within established protocols.
    • Provide direct physical and psychological nursing care in emergencies to maintain life and comfort.
    • Coordinate resident admissions and discharges, working with health agencies and families.
    • Assist in rehabilitation activities, such as range of motion exercises and ambulation support.
    • Provide preventive health care services, including recommendations on nutrition and illness prevention.
    • Educate residents and families on prevention techniques and maintaining general health.
  • Staff Development
    • Participate in planning and conducting in-service training on medical skills and procedures.
    • Engage in educational programs for residents, families, and the community.
    • Assist in developing annual OSHA and CDC training programs (e.g., hazard communication, TB management, bloodborne pathogens).
    • Support the development of advance directive training programs for staff and the community.
    • Attend continuing education programs to stay current in the profession and maintain licensure.
    • Assist the Infection Control Coordinator in developing a TB management program.
  • Safety and Equipment
    • Monitor inventory of medications, medical supplies, and equipment to ensure adequate supply.
    • Use personal protective equipment (e.g., masks, gowns, gloves, face shields) as appropriate.
    • Ensure availability of protective equipment for personnel exposed to blood or body fluids.
    • Assist in identifying and classifying job-related functions with potential exposure to blood/body fluids.
    • Ensure personnel receive training before performing tasks with exposure risks.
    • Participate in monitoring communicable and infectious diseases (e.g., TB) among residents and staff.
    • Enforce infection control procedures when isolation precautions are necessary.
    • Report hazardous conditions or equipment.
    • Ensure personnel use safety equipment (e.g., back braces, mechanical lifts) when moving or lifting residents.
  • Resident Rights
    • Maintain confidentiality of resident care information.
    • Monitor care provided to residents to ensure they are treated with kindness, dignity, and respect.
    • Report and document resident complaints or grievances regarding medical care.
    • Ensure nursing care is provided in privacy and staff knock before entering resident rooms.
    • Educate nursing staff on resident rights, including the right to refuse treatment.
    • Report and investigate allegations of resident abuse or misappropriation of property.
    • Honor resident refusals of treatment in accordance with facility policies on advance directives.
    • Review advance directive policies with residents and/or their representatives.

Working Conditions

  • Works in office areas, nursing service areas (e.g., drug rooms, nurses’ stations, resident rooms).
  • Moves intermittently during working hours and is subject to frequent interruptions.
  • Interacts with residents, personnel, visitors, and government agencies under various conditions.
  • May encounter hostile or emotionally upset residents, family members, or visitors.
  • Communicates with medical staff, nursing personnel, and department supervisors.
  • Works beyond normal hours, including weekends and holidays, and is on call 24/7.
  • May be called back during emergencies (e.g., severe weather, evacuations, post-disaster).
  • Participates in community/civic health matters and continuing education programs.
  • Exposed to potential hazards, including falls, burns, odors, infectious waste, diseases, and air contaminants (e.g., dust, disinfectants, tobacco smoke).

Schedule

Full-time night shift with 12-hour shifts.
