  • Complete the questionnaire thoroughly and submit all requested information.
  • Upload relevant certificates and/or screenshots verifying the firm's status.
  • Provide details on any judgments, arbitrations, liens, insurance claims, or lawsuits within the past 5 years.
  • Answer questions regarding criminal background information, including convictions and charges.
  • Specify insurance coverage limits and willingness to purchase additional coverage.
  • Detail the firm's health and safety programs, including written policies, site-specific plans, inspections, investigations, and training.
  • Describe the firm's safety practices, such as drug and alcohol screening, first aid, and safety meetings.
  • Provide information on fall protection, electrical safety, confined space training, and other relevant safety measures.
