Patient Services Coordinator LPN
About the role
The Patient Services Coordinator, LPN/LVN acts as a vital link between clinical staff and patients, along with their families and caregivers. They are responsible for scheduling, organizing, and managing all aspects of a patient's healthcare maintenance and treatment. This role involves making decisions regarding own work approach/priorities and following direction, with work being managed and often guided by precedent and/or documented procedures/professional standards with some interpretation.
Responsibilities
- Explain complex medical terminology in simple, understandable terms to patients and their families.
- Facilitate clear communication between patients and the medical team, ensuring that patients' concerns and questions are effectively conveyed.
- Schedule, organize, and manage all aspects of a patient's healthcare maintenance and treatment.
- Manage schedules for all patients, including supervisory visits.
- Reschedule declined or missed visits and process reassigned and rescheduled visits.
- Prepare and edit weekly agent schedules, perform initial review for productivity/geographic issues, and forward for approval.
- Absorb and assist with weekly case conferences.
- Receive calls from field staff and refer clinical questions to the Branch Director as necessary.
- May perform patient visits and/or participate in on-call rotation.
- Maintain the client hospitalization log, including entering coordination notes.
- Send electronic logs to all office, field, and sales staff, including receiving lab reports, assessing for normality, and distributing as appropriate.
- Verify visit paper notes in the scheduling console as needed.
- Run all scheduling reports, including Agent Summary Report and Missed Visits Done on Paper Report.
- Absorb and assist with the internal transfer of patients between branch offices.
Requirements
Required:
- A Licensed Professional Nurse or a Licensed Vocational Nurse licensed in the state in which they practice.
- One year of Home Health experience.
- Prior packet review / QI experience.
- Coding certification.

- Current license to practice as a Licensed Professional Nurse or a Licensed Vocational Nurse in the state in which they practice.
- One year of Home Health experience.

Pay
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
- $48,900 - $66,200 per year
