Patient Services Coordinator Hospice - Full-time

Responsibilities
- Schedule patients to branch field clinicians.
- Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers in order to maintain proper care coordination and continuity of care.
- Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.
Qualifications
- Must possess a high school diploma or equivalent.
- Must either:
- Be a licensed practical or vocational nurse in the state in which they currently practice, with at least one year of clinical experience in a healthcare setting; or
- Have at least one year of home health, hospice, or pediatric experience within the last 24 months, and have a demonstrated understanding of staffing and scheduling requirements related to home care services.
- Must have basic demonstrated technology skills, including operation of a mobile device.
Previous experience in home health, hospice, or pediatrics is preferred.
Requirements
- Must possess a valid state driver license.
- Must maintain automobile liability insurance as required by law.
- Must maintain dependable transportation in good working condition.
- Must be able to safely drive an automobile in all types of weather conditions.
For employees located in Oregon, requirements related to driving are not applicable unless employee has a clinical license.
