Patient Services Coordinator, Hospice - Full Time

What You’ll Do

  • Schedule and coordinate patient visits across hospice clinicians to ensure timely, efficient care
  • Collaborate with nurses, social workers, chaplains, and leadership to support daily care coordination
  • Maintain accurate documentation and workflow completion within the EMR system
  • Respond to changing patient needs and scheduling challenges with urgency and problem-solving
  • Utilize EMR and reporting tools to track productivity, coverage, and scheduling optimization
  • Monitor schedules and follow up on missed, incomplete, or delayed visits
  • Support quality initiatives, including participation in QAPI programs
  • Communicate effectively with patients, families, clinicians, and referral partners

What We’re Looking For

  • A high school diploma or equivalent required
  • 1+ year of administrative, scheduling, or office experience (healthcare or hospice preferred)
  • Strong organizational and multitasking skills
  • Experience working with electronic medical records (EMR) or scheduling systems preferred
  • Excellent communication and interpersonal skills
  • Ability to prioritize, adapt, and problem-solve in real time
