What You’ll Do
- Schedule and coordinate patient visits across hospice clinicians to ensure timely, efficient care
- Collaborate with nurses, social workers, chaplains, and leadership to support daily care coordination
- Maintain accurate documentation and workflow completion within the EMR system
- Respond to changing patient needs and scheduling challenges with urgency and problem-solving
- Utilize EMR and reporting tools to track productivity, coverage, and scheduling optimization
- Monitor schedules and follow up on missed, incomplete, or delayed visits
- Support quality initiatives, including participation in QAPI programs
- Communicate effectively with patients, families, clinicians, and referral partners
What We’re Looking For
- A high school diploma or equivalent required
- 1+ year of administrative, scheduling, or office experience (healthcare or hospice preferred)
- Strong organizational and multitasking skills
- Experience working with electronic medical records (EMR) or scheduling systems preferred
- Excellent communication and interpersonal skills
- Ability to prioritize, adapt, and problem-solve in real time
