About the role

The Patient Services Coordinator is responsible for scheduling visits and communicating with field staff, patients, physicians, and other stakeholders. This role also assists with day-to-day office and staff management.

Responsibilities

  • Manages schedules for all patients, including editing schedules for agents calling in sick to ensure timely patient reassignment.
  • Updates agent unavailability in the worker console.
  • Initiates infection control forms as needed and sends completed “Employee Infection Report” to HRD for upload in the worker console.
  • Serves as backup during lunch hours and busy periods, including receiving calls from field staff and assisting with weekly case conferences. Refers clinical questions to the Branch Director as necessary.
  • Maintains the client hospitalization log, including entering coordination notes and sending the electronic log to all office, field, and sales staff.
  • Completes requested schedules as tasks appear on the action screen, ensuring staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is scheduled in conjunction.
  • Schedules discharge visits, OASIS Collection, or recertification visits following case conferences when tasks appear on the action screen.
  • Schedules TIF OASIS collection visits and deletes remaining schedules.
  • Reschedules declined or missed visits (if appropriate).
  • Processes reassigned and rescheduled visits.
  • Ensures supervisory visits are scheduled.
  • Runs all scheduling reports, including the Agent Summary Report and Missed Visits Done on Paper Report.
  • Prepares weekly Agent Schedules, performs initial reviews for productivity/geographic issues, and forwards schedules to the Branch Director for approval prior to distribution.
  • Verifies visit paper notes in the scheduling console as needed.
  • Assists with internal transfer of patients between branch offices.
  • If clinical, receives lab reports, assesses for normality, faxes copies to doctors, provides copies to Case Managers, and routes to the Medical Records Department. Initiates Employee/Patient Infection Reports as necessary.
  • If clinical, may perform patient visits and/or participate in on-call rotation.

Requirements

  • At least 1 year of home health experience.
  • Excellent communication skills and the ability to interact well with a diverse group of individuals.
  • Strong organizational skills and the ability to manage and prioritize multiple assignments.

Qualifications

  • Prior packet review/QI experience preferred.
  • Coding certification is preferred.

Pay

The compensation range below reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location, and individual pay will vary based on demonstrated job-related skills, knowledge, experience, education, and certifications.

$40,000 - $52,300 per year
