Location: On-site / New Milford, CT Office
Responsibilities
- Scheduling & Care Coordination
- Maintain accurate patient schedules, ensuring all ordered visits are appropriately assigned across disciplines
- Schedule all Start of Care (SOC) and Resumption of Care (ROC) visits in the scheduling system
- Assign and maintain a primary case manager for each patient
- Coordinate clinician coverage for absences, including vacations and sick time
- Communicate with patients and families regarding visit schedules and changes
- Episode Management & Compliance
- Ensure visits are completed timely to support billing, compliance, and quality outcomes
- Reconcile scheduled visits against physician orders to ensure regulatory adherence
- Monitor recertification timelines and provide weekly reminders to clinical staff
- Collaborate with clinical managers to support effective episode management
- Support scheduling strategies to minimize Low Utilization Payment Adjustments (LUPAs)
- Maintain working knowledge of Medicare guidelines and payment models
- Productivity & Operational Oversight
- Monitor clinician schedules to ensure productivity expectations are met
- Run and analyze reports related to visit completion, timeliness, and scheduling accuracy
- Identify scheduling gaps, inefficiencies, or risks and proactively resolve them
- Partner with leadership to ensure balanced coverage across service territories
- Communication & Collaboration
- Serve as a liaison between patients, families, clinicians, and leadership
- Communicate scheduling updates and important information to field staff
- Collaborate with interdisciplinary teams to ensure coordinated, patient-centered care
Requirements
- 2+ years of healthcare experience required, including medical terminology
- Bachelor's degree or licensed or experienced high-level functioning Medical Assistant, CNA, Coder, or other healthcare personnel experience
- Minimum one year of community health experience preferred, with a strong knowledge base of home health or hospice and associated regulations
- Knowledge of Medicare guidelines, episode management, and LUPA implications preferred
- Strong organizational, multitasking, and problem-solving skills
- Proficiency with EHR and scheduling systems
- Exceptional customer service skills including phone manner and ability to build relationships in a fast-paced environment

Pay
The pay range for this position is $25.00 to $30.00 per hour. The final salary will be determined based on the candidate's exact location, experience, and expertise related to the qualifications.
Schedule
Full-Time / Monday - Friday / 8:30am - 4:30pm
