Patient Services Coordinator

About the role

Responsible for providing compassionate care to patients and families in their homes. Works closely with healthcare providers to coordinate care plans.

Responsibilities

• Provide hospice care services including physical, emotional, and spiritual support to patients and families.
• Coordinate care with other healthcare professionals such as doctors, nurses, and social workers.
• Assist with daily living activities as needed.
• Offer bereavement support to families after the patient's passing.

Requirements

• Bachelor’s degree in a related field (nursing, social work, etc.) or equivalent experience.
• Current certification in hospice and palliative care.
• Experience working with terminally ill patients and their families.
• Excellent communication and interpersonal skills.

• Ability to handle sensitive and confidential information.
• Proficiency in Microsoft Office Suite.
