Schedule
Full time, day shift. Hours: Monday–Friday, 8:30 AM–5:00 PM with occasional weekend coverage.
Responsibilities
- Manage schedules for all patients and edit schedules for employees calling in sick, ensuring timely reassignment of patients.
- Complete requested schedules as tasks appear in workflow according to agency scheduling guidelines.
- Schedule discharge visits, OASIS collection, or recertification visits following case conferences when tasks appear on the action screen.
- Delete remaining schedules after TIF OASIS collection.
- Reschedule declined or missed visits when appropriate.
- Process reassigned and rescheduled visits.
- Ensure supervisory visits are scheduled according to agency protocol and regulatory mandates.
- Consider the entire episode of care and adjust service codes as needed related to discharge visits and supervisory visits.
- Update employee schedules daily, considering productivity and geographic issues, and discuss assignments with supervisors or managers for approval as directed.
- Assist with weekly coverage rotation to ensure scheduling functions are completed appropriately when staff are off.
- Facilitate communication between field staff, patients, and physicians to maintain proper care coordination and continuity of care.
- Maintain accurate records of all scheduled visits and communications.
- Perform other duties as assigned.
Requirements
- High school diploma or equivalent (required).
- 5+ years of home health experience and a good understanding of staffing and scheduling requirements related to home health from a field position perspective (required).
- Experience working in a Medicare-certified homecare agency (preferred).

Pay
Pay range: $16.63 – $26.60 per hour.
Location
770 W Granada Blvd, Ormond Beach, Florida 32174.
