Patient Portal Instructions

One of the features of our electronic medical records system is an online patient portal that allows you to fill out your medical history at home, prior to your appointment, which means you spend less time in our office.

The portal link is www.mypatientvisit.com/#/login?practiceID=PNYUCM.

Steps to Complete

• Enter your last name and a security code. If you do not have your security code, call our office at 478-953-1020 for assistance.
• After logging in, update your username and password.
• Navigate the portal using the five options at the top of the page:
  • Summary: Name/address information and option to change password.
  • Demographics: Fill out all requested information.
  • Insurance: Complete insurance information as shown on your most current insurance card.
  • Forms: Complete all nine forms pertaining to your personal and family health history. Use the "Next" button to proceed between forms.
• After completing the questionnaire, place your initials and date on the final page and proceed to the final step.
• Click the "Submit" button to save your information. Failure to do so will result in lost data, requiring you to fill out the forms again in our office.
• If you need to quit before completing all forms, click "Save and resume later." When you return, continue where you left off—do not restart the process.
