Patient Placement Coordinator RN - Full time Variable

About the role

Patient Placement Coordinator RN in Anaheim, CA. This position is Full-time and will work 10-hour, Variable shifts.

Responsibilities

• Evaluate and coordinate the patient intake process.
• Take and triage calls, and facilitate inbound and outbound transfers for PSJH facilities.
• Act as a liaison between physicians, hospitals and other care providers who seek to refer, schedule admission or transfer patients into the hospital.
• Ensure timely medical communication for patient referrals and transfers to/from the facility.
• Employ critical thinking to integrate objective data with knowledge gained from preliminary/initial information with the ongoing assessment process, including the objective and subjective experiences and patient health care history for optimal placement.
• Serve as a clinical resource for the team, providing guidance on clinical concerns, level of care determination, and regulatory considerations.
• Support onboarding and training of new staff, helping reinforce consistent practices across the teams.

Requirements

• California Registered Nurse License
• Bachelor’s Degree - Graduate from an accredited School of Nursing
• Minimum of 2-5 years nursing experience
• Working knowledge of Epic and Bed management system

Qualifications
