About the role

Atrium Health Transition Clinic is seeking a Nurse Navigator to support outreach, care coordination, and scheduling for its Transition Clinic. This is a fully remote, virtual role.

Responsibilities

• Receive and review referrals following recent patient discharges
• Determine appropriate next steps
• Schedule patients with a provider or connect them to the appropriate care team as needed
• Play a pivotal role in providing and coordinating comprehensive patient care through the nursing process
• Use evidence-based practice, clinical decision making, compassion, and skills communication
• Create the safest patient environment and the best patient experience across the continuum

Requirements

• Completion of an accredited or approved program in nursing
• Registered Nurse license issued by the state in which the teammate practices
• 3 years of nursing experience
• Prior Nurse Navigation experience preferred
• Ambulatory, triage, or care coordination/navigation experience preferred

Qualifications

Preferred: BSN
