Patient Navigator RN Dialysis
About the role
Join Baylor Scott & White Health's growing Outpatient Dialysis team supporting approximately 500 patients across three dialysis units. The RN Patient Navigator serves as a patient educator and advocate, helping patients successfully navigate their dialysis journey while supporting quality and patient experience initiatives.
Responsibilities
- Conduct new patient orientation and dialysis modality education.
- Provide targeted monthly patient education.
- Partner with leadership on quality improvement and patient experience projects.
- Build strong relationships with patients to improve outcomes.
- Care for patients in all aspects by evaluating their health based on doctor's orders and creating a healthcare plan.
- Schedule outpatient tests for diagnosis, stay in touch with the healthcare team.
- Visit doctors often, support patient-office communication.
- Collaborate with healthcare professionals for optimal patient care.
- Conduct follow-ups every 3 months and 1 year.
- Maintain up-to-the-minute and comprehensive nursing records, also updating patient details in the Patient Navigator database.
- Attend case conferences to expand your knowledge.
- Participate in community service events.
- Work together on improvement projects to enhance service for patients, families, and the healthcare system.
- Welcome patients and families, offer facility details, directions, and general information.
- Assist in guiding patients and families through the hospital, especially to hard-to-find places.
Requirements
- Registered Nurse (RN) with dialysis experience preferred.
- Strong patient education and communication skills.
- Experience with spreadsheets, dashboards, and data tracking.
- Team-oriented, flexible, and eager to learn.
- A Bachelor's degree.
- At least 3 years of relevant experience.
- Registered Licensed Nurse.
- Basic Life Support (BLS) certification or willingness to obtain within 30 days.
