Patient Navigator - Oncology Clinic (Oak Lawn)
About the role
The Patient Navigation position offers individualized assistance to patients, families and caregivers to overcome healthcare barriers. It involves coordinating appointments, monitoring patient satisfaction, advocating for patients, ensuring medical records are available, and arranging transportation assistance.
Responsibilities
- Schedule: Monday - Friday: 8a - 4:30p
- Play a vital role on the care team – supporting and connecting with patients, survivors, families and healthcare providers, to ensure a seamless, compassionate experience every day.
- Coordinate appointments with providers for a timely delivery of diagnostic and treatment services.
- Monitor patient satisfaction with their cancer experience and works with team(s) to address any identified issues or barriers.
- Advocates on behalf of the patient and empower the patient to self-advocate.
- Ensures that appropriate medical records are available at scheduled appointments.
- Arranges transportation assistance with available resources.
Requirements
- Bachelor's degree in healthcare related field preferred
- 2+ years of experience working in a medical setting with oncology patients and families.
- Ability to prioritize to work and work independently.
Qualifications
- Ability to work independently.
- Ability to handle stress and high workload volumes.
- Ability to communicate denial of services to members and providers.
Skills
- Ability to lift up to 35 pounds without assistance.
- Ability to lift over 35 pounds or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.

Compensation Base compensation listed within the listed pay range based on factors such as qualifications, skills, relevant experience, and/or training.
Premium pay such as shift, on call, and more based on a teammate's job.
Incentive pay for select positions.
