What You'll Be Doing
Actively identify individuals in need of support, assess their unique situations, and provide the consistent, compassionate follow-up required to guide them toward the right health services.
Offer informal counseling, health education, and genuine social support.
Help patients navigate the complex healthcare system by scheduling medical appointments, accompanying them to visits, and walking them through medical program applications.
Cultivate and sustain deep, collaborative relationships with internal medical teams and vital external community partners and referral sources.
Maintain precise, meaningful client records. By documenting every touchpoint, service, and breakthrough, you ensure continuity of care and help us track the lasting impact we are making on the community.
What You'll Need
- A high school diploma equivalency with 2 years of cumulative experience
- An associate's degree or bachelor's degree
- 4 years of applicable cumulative job-specific experience
- 2+ years of experience managing patient scheduling, care coordination, and thorough chart preparation
Education
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
