Full Time, Day Shift: 8am - 4:30pm, Monday - Friday
Responsibilities
- Coordinates patient care and maximizes patient satisfaction and experience.
- Partners with care providers to serve as a liaison to ensure effective communication with patients.
- Provides discharge information.
- Partners with providers to ensure a smooth transition at discharge to home or other facilities as required.
- Ensures additional resources are made available for the patient as needed.
- Effectively collaborates with other healthcare providers and non-Geisinger employees in coordinating and implementing the patient's plan of care.
- Maintains systems and processes to assure timely follow-up and surveillance of a defined group of patients.
- Serves as liaison with clinical and administrative personnel.
- Establishes point of contact for referring providers to create immediate access to provider templates.
- Develops and implements efficient patient navigational procedures.
- Investigates patient concerns including billing, appointment access, perception of care, physician issues, and communication problems.
- Develops, maintains, and audits standard operating procedures of patient access and utilization process.
- May provide administrative support as needed.
Work is typically performed in a clinical and office environment.
Requirements
- Minimum of 2 years related work experience
- High School Diploma or GED (Required)
- Associate's Degree (Preferred)
