RN Patient Navigator - Neurosurgery

About the Role

The role is based in Department 37150 at Wake Forest Baptist Medical Center's Spine Center in Clemmons Med Plz. It is a full-time position with a schedule of Monday to Friday, 0800-1700, and no weekends or holidays.

Responsibilities

• Ongoing evaluation and quality improvement of the Navigator Program, tracking all activity as defined by departmental and program standards.
• Ensuring quality patient care by adhering to WFBMC policies, involving patients and their families in identifying needs, creating a plan to meet those needs, and communicating the plan across the continuum of care.
• Identifying barriers to efficient and effective management of patient care and seeking strategies to remove them.
• Implementing outreach efforts to establish and maintain positive working relationships with key community agencies.
• Collecting and maintaining up-to-date and accurate program data and documentation according to program standards.
• Delivering reports that demonstrate outcomes and performance improvement activities, adhering to established productivity benchmarks.
• Providing chronic care management through motivational interviewing and health coaching.
• Functioning in a professional practice model through active participation in shared governance and personal responsibility for professional growth.
• Providing age/developmentally appropriate patient care in accordance with Age-Specific guidelines.
• Serving as a liaison between the provider and patient between clinic visits, communicating clinical concerns, and providing transitional care.
• Identifying and addressing barriers to care and assisting with identifying community resources.
• Meeting weekly with the primary medical team to discuss current patients and identify new patients at risk for services.
• Participating in Transitional and Supportive Care rounds to identify patients who are candidates for services.

Requirements

• Graduation from an accredited School of Nursing (BSN preferred).
• Two years' experience as a Registered Nurse.
• Case management experience preferred.
• Bilingual preference with fluency in Spanish.
• Current license as a Registered Nurse (RN) in the applicable state.

Pay

$38.20 - $57.30 per hour, with opportunities for premium pay, incentive pay, and annual increases based on performance.
