About the role
Bridges care gaps: connects pregnant patients to prenatal visits, mental-health support, and social services to reduce missed appointments. Empowers families: educates patients on pregnancy milestones, warning signs, and self-advocacy to build confidence. Reduces stress: helps families navigate insurance, transportation, and medical paperwork so they can focus on health. Builds trust: serves as a consistent, supportive point of contact between the patient and the clinical care team.
Requirements
- High School diploma or equivalency with 2 years of cumulative experience or Associate’s degree/Bachelor’s degree or 4 years of applicable cumulative job-specific experience
Preferred Qualifications
- Associate’s degree in Social Work or Psychology
- Familiarity navigating the Medicaid system
- Familiar with community resources
