Patient Navigator Maternal Health

Location: Tulsa, OK – Ascension St. John Medical Center, Total Quality Management

Schedule: Full-time, Monday-Friday, 8:00 AM–5:00 PM

Responsibilities

• Bridges care gaps: Connects pregnant patients to prenatal visits, mental-health support, and social services to reduce missed appointments.
• Empowers families: Educates patients on pregnancy milestones, warning signs, and self-advocacy to build confidence.
• Reduces stress: Helps families navigate insurance, transportation, and medical paperwork so they can focus on health.
• Builds trust: Serves as a consistent, supportive point of contact between the patient and the clinical care team.

Requirements

• High School diploma equivalency with 2 years of cumulative experience OR Associate’s degree/Bachelor’s degree OR 4 years of applicable cumulative job-specific experience.

Qualifications

• Associate degree in Social Work or Psychology preferred.
• Familiarity navigating the Medicaid system.
• Familiar with community resources.
