Responsibilities
- Bridges care gaps: Connects pregnant patients to prenatal visits, mental-health support, and social services to reduce missed appointments.
- Empowers families: Educates patients on pregnancy milestones, warning signs, and self-advocacy to build confidence.
- Reduces stress: Helps families navigate insurance, transportation, and medical paperwork so they can focus on health.
- Builds trust: Serves as a consistent, supportive point of contact between the patient and the clinical care team.
Requirements
- High School diploma equivalency with 2 years of cumulative experience OR Associate’s degree/Bachelor’s degree OR 4 years of applicable cumulative job-specific experience.
Qualifications
- Associate degree in Social Work or Psychology preferred.
- Familiarity navigating the Medicaid system.
- Familiar with community resources.
