The Patient Navigator position at Pinnacle will allow you to engage with the patient throughout their journey while working closely with a multidisciplinary team to provide excellent patient care. We are seeking a Patient Navigator to join our dedicated team at Pinnacle Fertility - Virginia located in Arlington, VA. This is a full‑time, on‑site position.

Responsibilities

• Serve as the primary point of contact for patients, guiding them through their treatment plan and providing education on procedures, testing, and follow‑up care.
• Coordinate and schedule patient appointments, tests, and referrals to ensure a smooth continuum of care.
• Assist patients with understanding their medical care, insurance coverage, and available financial resources.
• Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless patient care coordination.
• Maintain accurate and up‑to‑date patient records, including documentation of interactions and care plans in the electronic medical record (EMR) system.
• Provide emotional support and resources to patients and their families, addressing any concerns or barriers to care.
• Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.
• Educate patients on treatment protocols, self‑care strategies, and medication usage.
• Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
• Advocate for patients’ needs and help resolve any issues or barriers to care.
• Assist with after‑hours call coverage on a rotating schedule, ensuring patient needs are addressed appropriately.
• Other duties as assigned.

Requirements

Education: High school diploma required. Bachelor’s degree preferred.

Experience: Minimum of 2 years of experience in a clinical healthcare setting, including patient care coordination, patient navigation, or case management.

Skills

• Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
• Excellent communication, organizational, and problem‑solving skills.
• Proficiency in using electronic medical records (EMR) and other healthcare software.
• Bilingual skills are a plus.

Pay

Hourly Rate: $23.00 - $26.00 per hour (Final offers based on experience, skills, and qualifications).
