Patient Navigation
- Guide patients through various processes, including check-in, intake, and check-out procedures.
- Schedule and coordinate follow-up appointments, ensuring timely and accurate bookings.
- Remind patients of upcoming appointments via phone calls.
- Handle phone calls to and from patients, addressing inquiries and providing information as needed.
- Communicate referral information to patients, ensuring they understand the next steps in their care.
- Affirmatively assist patients with understanding their insurance benefits and coverage.
Patient Identification and Paperwork
- Verify patient identification to ensure accurate and secure records.
- Affirmatively assist patients with completing intake paperwork and applications, including the sliding fee scale program application.
- Educate patients on the importance and completion of required documents.
- Verify patient insurance eligibility and navigate insurance portals to obtain necessary information.
Other Duties
- Maintain and ensure patient confidentiality at all times.
- Maintain a courteous and professional demeanor at all times; greet individuals and patients as they enter and exit the clinic; facilitate appropriate communication and interpersonal skills with all staff members, volunteers, and patients.
- Maintain a safe, secure, and healthy work environment by establishing and following standards and procedures; comply with legal regulations.
- Ensure front office and clinic policies and procedures (as applicable) are followed at all times.
Requirements
- A High School Diploma or equivalent – required.
- Community Health Worker (CHW) Certification – preferred.
- Previous experience in a healthcare or administrative role – preferred.
- Familiarity with electronic medical records (EMR) systems and insurance portals – preferred.
- Spanish fluency – required.
- Reliable Transportation – required.

Typical Physical Demands
- Occasional standing, walking, lifting, reaching, kneeling, bending, stooping, pushing, pulling and lifting and/or moving up to 25 pounds.

- Good reading eyesight and vision for close work.
- This role involves working in varying weather conditions, both Summer and Winter.
