Patient Navigator

About the Role

The Patient Navigator is a healthcare professional that provides supportive counseling and referral information for patients and families, performs complete psychosocial assessments, and conducts mental health and substance abuse screenings. The Navigator is committed to removing patients' barriers to care by identifying critical resources, guiding them through healthcare services and systems, and promoting patient health. This role demonstrates understanding of community resources and collaborates effectively with agencies and hospital personnel.

This position supports organizational goals by providing quality customer service, participating in performance improvement efforts, and demonstrating a commitment to teamwork and cooperation.

Responsibilities

• Perform complete psychosocial assessments and mental health/substance abuse screenings.
• Identify and connect patients with critical resources to remove barriers to care.
• Write medical record notes within 48 hours.
• Build constructive relationships with community service professionals.
• Communicate and collaborate effectively with agencies and hospital personnel.
• Demonstrate competencies as established on the Assessment and Evaluation Tool for this position.

Requirements

• Bachelor’s degree in Social Science (required).
• Minimum of 2 years’ experience in case management (required).
• Excellent written and verbal communication skills (required).
• Ability to work with diverse populations (required).
• Bilingual (preferred).
• Proficiency in Clinical Applications (preferred at time of hire; required within 30-60 days of hire).

Work Environment

Potential for exposure to hazards and risks of the ambulatory and hospital environment, including exposure to infectious disease, hazardous substances, and potential injury.
