Job Duties
- Provides discharge information.
- PARTNERS WITH PROVIDERS TO ENSURE A SMOOTH TRANSITION AT DISCHARGE TO HOME OR OTHER FACILITIES AS REQUIRED.
- ENSURES ADDITIONAL RESOURCES ARE MADE AVAILABLE FOR THE PATIENT AS NEEDED.
- EFFECTIVELY COLLABORATES WITH OTHER HEALTHCARE PROVIDERS AND NON-GEISINGER EMPLOYEES IN COORDINATING AND IMPLEMENTING THE PATIENTS PLAN OF CARE.
- MaintAINS SYSTEMS AND PROCESSES TO ASSURE TIMELY FOLLOW-UP AND SURVEILLANCE OF A DEFINED GROUP OF PATIENTS.
- SERVES AS LIASON WITH CLINICAL AND ADMINISTRATIVE PERSONNEL.
- ESTABLISHES POINT OF CONTACT FOR REFERRING PROVIDERS TO CREATE IMMEDIATE ACCESS TO PROVIDER TEMPLATES.
- DEVELOPS AND IMPLEMENTS EFFICIENT PATIENT NAVIGATIONAL PROCEDURES.
- INVESTIGATES PATIENT CONCERNS INCLUDING BILLING, APPOINTMENT ACCESS, PERCEPTION OF CARE, PHYSICIAN ISSUES AND COMMUNICATION PROBLEMS.
- DEVELOPS, MAINTAINS, AND AUDITS STANDARD OPERATING PROCEDURES OF PATIENT ACCESS AND UTILIZATION PROCESS.
- MAY PROVIDE ADMINISTRATIVE SUPPORT AS NEEDED.

- MINIMUM OF 2 YEARS RELATED WORK EXPERIENCE (REQUIRED)
Qualifications
- EDUCATION: HIGH SCHOOL DIPLOMA OR EQUIVALENT (GED) - (REQUIRED), ASSOCIATE'S DEGREE - (PREFERRED)
