About the role

The Patient Flow Coordinator (PFC) functions as a hybrid position, extending both Patient Flow Coordinator (PFC) and case management (CM) capabilities into off-shift hours. The primary focus is to manage Fox Chase Transfer Center (FCTC) operations, finalize case management details for evening and weekend discharges, and cover triage phone calls.

Responsibilities

• Manage Fox Chase Transfer Center (FCTC) operations
• Finalize case management details for evening and weekend discharges
• Cover triage phone calls
• Be the first point of contact for outside patient transfers and internal admissions
• Put the transferring physician/FCCC clinic/DRU physician in contact with the appropriate FCCC accepting physician
• Acktively manage beds throughout the institution
• Finalize case management-related disposition
• Serve as a resource for transfer-related insurance issues
• Answer incoming triage calls

Requirements

• Bachelor's Degree in Nursing
• 3 years experience in acute care nursing
• 1 year experience in case management
• PREFERRED: 3 years experience in oncology nursing
• PA Registered Nurse License
• Basic Life Support
• OCN - Oncology Certified Nurse
• Multi-State Compact RN License
