Patient Coordinator, Multi-Specialty, Full-Time, Days
About the role
Serves as primary non-clinical contact for care coordination for patients, families and referring providers
Coordinates, schedules and manages appointments for consultation, education and testing
Liaises with clinical and non-clinical teams to confirm and validate schedules and ensure patients are on schedule throughout their visit
Provides advance notice to patients and referring providers of required follow-up testing and coordinates the care as needed
Collaborates with various departments to ensure billing information is accurate and current
Distributes program materials, welcome packets, educational materials, itineraries and other collateral as needed
Coordinates pre-visit assessments/planning/questionnaires
Facilitates required referrals, pre-certification, and authorizations as required
Screens patients for supportive care services if needed (i.e., dietician, social work and financial counseling)
Directs patients and families to available community resources and supportive services
Assists patients to overcome barriers to care, i.e., transportation, childcare, financial counseling

Requirements
High school diploma or equivalent
Preferred Qualifications: Medical terminology certification
Bachelor’s degree, 2 years’ experience in hospital or medical office
Microsoft, Epic, Appointment Scheduling, Insurance Verification, Pre-certification, Prior Authorization, billing & collections
Excellent verbal and written communication

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis.
Pay
Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity.
