PATIENT CARE - STEP DOWN NAVIGATOR

Responsibilities

The Step-Down Navigator is responsible for meeting with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. The Step-Down Navigator will communicate eligibility to key members of senior leadership & associated departments and routinely collect and enter data, specific to Outpatient eligibility, into Midas for trending report purposes. They will have a thorough understanding of the Step-Down Playbook, implementing components of the Playbook as needed. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred.

• Meet with eligible patients to discuss the benefits of step-down services as part of their continuum of care treatment.
• Communicate eligibility to senior leadership and associated departments.
• Cookordination between departments to ensure a smooth transition for patients.
• Conduct patient therapy groups

Requirements

• Education: High School graduation or GED required.
• Experience: Minimum two (2) years’ experience in an inpatient psychiatric/mental health setting.
• Licensure: Peer Support Certified, preferred. Current CPR, Physical and Verbal Crisis Intervention Certification, preferred.
