Responsibilities

  • Review medication orders for appropriateness and adjust regimens based on protocols and interdisciplinary team consultations.
  • Monitor medication therapy for potential or actual drug interactions and outcomes, collaborating with healthcare providers.
  • Participate in interdisciplinary patient care rounds and oversee pharmacy resident and student participation.
  • Perform medication histories, document medication reconciliation across patient transitions, and facilitate the discharge process.
  • Ensure compliance with inpatient drug formularies and complete medication profiles, considering various factors.
  • Coordinate and distribute medications, ensuring accurate and timely delivery, and communicate pending issues.
  • Provide direct supervision of certified pharmacy technicians, pharmacist interns, and students, and provide drug information/education.
  • Manage safety precautions, including handling hazardous materials and managing blood-borne pathogens.
  • Engage in quality improvement initiatives, reporting adverse drug events and ensuring compliance with regulations.
  • Offer in-services and lectures to health care team members and conduct drug information and disease prevention programs.

Requirements

  • Education: Bachelor’s degree in Pharmacy or PharmD required.
  • Experience: Two years of health care pharmacy experience preferred.
  • Licenses/Certifications: Current active Pharmacy Licensure (PH) for work state required; BLS, ACLS, PALS preferred; residency training program and certification preferred.

  • Manage multiple concurrent activities.

Physical Requirements

  • Weight Demands: Medium Work (up to 50 pounds).
  • Physical Activity: Occasional (carrying, lifting, pulling/pushing, stooping/bending, twisting).
  • Equipment/Machinery/Tools: Keyboarding/typing, reaching, grasping, speaking/talking, standing, walking.
