Patient Care Coordinator RN

The Patient Care Coordinator (PCC) manages and coordinates care for hospitalized patients, collaborating with physicians, nursing staff, and community providers to ensure seamless care transitions and prevent readmissions. Responsibilities include conducting initial assessments within 24 hours of admission, documenting discharge plans, participating in daily rounds, and managing care progression to avoid delays. The PCC facilitates referrals for post-acute services, monitors barriers to discharge, and ensures follow-up appointments are scheduled. They also coordinate with social workers for long-term care and assist with regulatory compliance and quality improvement initiatives. Qualifications include at least one year of experience in acute care nursing, an associate degree in nursing, and valid RN licenses for relevant states or a compact license. Required skills include electronic medical record proficiency, effective communication, critical thinking, time management, and teamwork. Preferred candidates have recent acute care or case management experience and a BSN.
