About the role
The Patient Care Coordinator oversees care management for inpatient populations, collaborating with physicians, nursing, social workers, and community providers to ensure smooth care transitions, timely discharges, and the prevention of readmissions.
Responsibilities
- Conduct initial assessments within 24 hours of admission
- Document and communicate discharge plans
- Participate in daily rounds
- Coordinate post-acute care referrals
- Manage delays or barriers to discharge
- Ensure follow-up appointments are scheduled
- Maintain compliance with safety and regulatory policies
- Participate in quality improvement initiatives
- Foster effective communication with patients, families, and healthcare teams
Qualifications
- At least one year of acute care nursing experience
- Associate degree in nursing
- Valid RN license with multi-state credentials
- BLS certification
- Experience with electronic medical records
- Strong communication, organizational, and critical thinking skills
- Prior experience in acute care, case management, or home health is preferred
- BSN considered advantageous
