Patient Care Coordinator RN

About the role

The Patient Care Coordinator oversees care management for the inpatient population, collaborating with physicians, nursing, and community providers to ensure seamless patient transitions and prevent readmissions.

Responsibilities

• Conducting initial assessments within 24 hours of admission to identify discharge needs.
• Documenting assessments and communicating outcomes for care planning.
• Participating in daily rounds to monitor patient progress toward discharge.
• Coordinating discharge plans, referrals, and follow-up appointments with appropriate services.
• Managing delays in care and barriers to discharge, providing solutions, and escalating issues as needed.
• Collaborating with social workers and vendors for post-acute care arrangements.
• Ensuring compliance with safety policies, regulatory requirements, and quality initiatives.
• Maintaining professionalism and teamwork in a fast-paced environment.

Qualifications

• At least one year of experience in acute medical, surgical, emergency, or critical care nursing.
• Associate Nursing degree; RN license (Virginia, Maryland, D.C., or compact) within six months of hire.
• Basic Life Support certification.
• Experience with electronic medical records and understanding of healthcare reimbursement systems.
• Strong communication, time management, critical thinking, and teamwork skills.

Preferred qualifications

• Recent acute care or case management experience.
• BSN.
