Patient Care Coordinator II (Medical Assistant), GI Services South Bay – Full Time, Days (08HR)
1.0 FTE, full-time, 8-hour day shift, onsite in San Jose, California.
About the role
As a foundational level Patient Care Coordinator II (PCC II), you will learn and apply job skills, policies, and procedures to complete moderate assignments and tasks while exercising judgment within defined guidelines. This role facilitates smooth day-to-day clinic operations and develops the skills needed to progress within our care delivery team.
Responsibilities
- Develop clinical expertise in assigned specialty areas or clinics.
- Complete disability paperwork, maintain temperature logs, and follow assigned protocols to ensure workplace safety and infection prevention.
- Perform patient care duties, including measuring vital signs, weight, height, rooming, drawing labs, assisting providers with medication reconciliation, and other special departmental tasks such as EKG and ear lavage.
- Handle clinical administration tasks, including check-in/out, virtual rooming for providers, vaccine administration, point-of-care testing, and specimen collection.
- Manage weekly clinic preparation, stocking clinic areas, ordering medications and supplies, and ensuring timely completion of work assignments.
- Handle patient phone calls, prescription refills, and clinic support operations; manage CRM/In-Basket and serve as a liaison between providers, patients, and staff.
Qualifications
Education: High School Diploma or GED equivalent AND one of the following:
- Certificate of completion or official transcript from a medical assisting program.
- Completion of medical assistant training by a licensed physician or podiatrist.
- Certification as a medical assistant through a Medical Board of California–approved certifying organization.
Experience: At least one year of overall experience (including external experience).
Skills
- Knowledge of and ability to use and apply medical terminology.

- Ability to speak and write effectively at a level appropriate for the job.
- Ability to solve problems and identify solutions.

- Familiarity with home health, practice care coordination, and clinical care skills.
- Business communication skills, including legible handwriting, grammar, syntax, and concise clinical reporting.
- Ability to learn and update clinical knowledge regularly to adapt to a changing healthcare environment.
- Understanding of population health, disease population concepts, inventory methods, and customer service practices.
Licenses and Certifications
- Current American Heart Association Certification for Basic Life Support for Healthcare Providers.
Pay
Generally starting at $33.22 - $37.39 per hour. The salary of the finalist selected for this role will be set based on factors including internal equity, experience, education, specialty, and training.
