About Zest HealthZest Health is redefining care for the 40M+ Americans living with chronic dermatologic conditions such as eczema and psoriasis. Through our virtual dermatology clinic, we provide patients with ongoing access to specialized care—not just during office visits, but throughout the course of their disease. Our model combines dermatology expertise, longitudinal monitoring, patient education, medication support, and technology to improve outcomes while making specialty care more accessible and affordable. We are building a new model for value-based specialty care and are looking for thoughtful, mission-driven people to help us scale it. About the RoleZest is hiring a Patient Care Coordinator (PCC) to join our clinical team. PCCs are the connective tissue between patients, providers, pharmacies, and the broader care model. This role goes well beyond appointment scheduling. What We’re Looking ForCertified Medical Assistant or comparable healthcare experience preferred.Excellent written and verbal communication skills, with the ability to communicate clearly and empathetically with patients.Prior experience in a remote, virtual care delivery system preferred.Experience in dermatology, chronic disease management, or specialty medication workflows is strongly preferred.Experience with prior authorizations, specialty pharmacies, biologic medications, copay assistance, or patient assistance programs is a plus.High attention to detail and a strong sense of ownership for closing loops and following tasks through to completion.Comfort working independently and collaboratively in a fast-moving, remote clinical environment.Working knowledge of medical terminology and healthcare workflows.Ability to recognize when a patient concern requires escalation to a licensed clinician.Bilingual English/Spanish is strongly preferred. What Success Looks LikeA successful PCC makes it easier for patients to stay connected to their care. You anticipate what needs to happen next, follow through until it does, and make sure patients do not fall through the cracks. You build relationships with patients while maintaining clear clinical boundaries. You understand that small operational details—a missed refill, an incomplete lab, an unanswered message, or a patient who quietly disengages—can meaningfully affect chronic disease outcomes. Most importantly, you see care coordination not as an administrative function, but as an essential part of delivering excellent longitudinal care. Zest Health is an equal opportunity employer and encourages candidates from all backgrounds to apply.
