We need to output HTML fragment with sections only if content present. Use

for headings: About the role, Responsibilities, Requirements, Qualifications, Skills, Benefits, Pay, Schedule. We need to decide which sections have content. From posting: - About the role: we have mission description and purpose of position. Could be "About the role" section. - Responsibilities: list of key responsibilities. - Requirements: maybe "Qualifications and Education Requirements" and "Essential Skills And Qualifications" etc. We need to separate Qualifications and Skills? The instruction says use headings for those categories if content present. We have "Qualifications and Education Requirements" and "Essential Skills And Qualifications" and "Knowledge, Skills, And Abilities". Also "Preferred Skills". We could combine into Qualifications and Skills sections. - Benefits: not mentioned explicitly; only compensation range, professional development opportunity. Benefits not given. So we should not create Benefits section unless posting actually states them. It mentions "Professional Development: Opportunity to collaborate..." that's not a benefit like health insurance. Probably not include Benefits. - Pay: compensation range given: $25 - $28 per hour. So we can include Pay section. - Schedule: not mentioned besides "Fully remote position" and "California based preferred, PST time zone work is required." That's schedule? Could be considered schedule (time zone). But schedule usually refers to hours/shift. Not given explicit. Might omit Schedule. - Also "Working Conditions" etc. Could be part of About the role or Requirements? Not needed. We must not repeat job title as heading. No Job ID. We must preserve all substantive information. So we need to include mission description, purpose, highlights, responsibilities, qualifications, essential skills, knowledge/skills/abilities, preferred skills, working conditions, physical requirements maybe? Those are substantive. Where to place them? Could be under About the role (mission, purpose), Responsibilities (list), Qualifications (education, experience), Skills (essential skills, knowledge, abilities, preferred), Pay (compensation), maybe Working Conditions as its own section? Not in allowed headings list. Allowed headings: About the role, Responsibilities, Requirements, Qualifications, Skills, Benefits, Pay, Schedule. So we need to map content to these. We could put "Requirements" for education and experience. "Qualifications" maybe for essential skills? But they'd overlap. Perhaps better: Use "Requirements" for mandatory qualifications (education, experience). Use "Skills" for essential skills, knowledge, abilities, preferred skills. Use "About the role" for mission, purpose, highlights, working conditions? Working conditions maybe not needed but could be part of About the role. We must not create sections with filler. So ensure each heading has substantive content. Let's draft:

About the role

Wellness Equity Alliance is a national multidisciplinary health organization that designs and delivers integrated, community-based care for populations most impacted by health inequities through mobile and field-based models, providing medical care, behavioral health services, substance use treatment, harm reduction, and care coordination in nontraditional settings such as encampments, schools, reentry sites, and rural communities as well as with sovereign tribal nations. Grounded in trauma-informed, culturally responsive, and data-driven practices, WEA combines clinical expertise, lived experience, and advanced population health analytics to reduce barriers to care, improve continuity, and strengthen local systems. We have partnered with more than 60 public agencies, managed care plans, and community-based organizations across the U.S. to implement scalable, sustainable programs that are advancing health equity and improving outcomes for historically marginalized populations. We are known as Renegades, Rebels, Disruptors and Dreamers. If that sounds like you, we want you on our team.

The Patient Care Coordinator serves as a key member of the Wellness Equity Alliance (WEA) care team, responsible for conducting telephonic outreach to prospective CalAIM members and connecting eligible individuals to WEA's Enhanced Care Management (ECM) and Community Supports programs. This position focuses exclusively on remote outreach activities, engaging patients through phone calls, educating them on available services, and facilitating enrollment into appropriate programs. The ideal candidate is compassionate, organized, comfortable making a high volume of outbound calls, and passionate about improving health outcomes for underserved populations.

Key Highlights
Compensation: The compensation range for this role is $25 - $28 per hour, with final compensation determined based on experience, qualifications, and role scope.
Work Location & Expectations: California based preferred, PST time zone work is required.
Professional Development: Opportunity to collaborate with cross-functional leaders across Behavioral Health, Medical, Street Medicine, Public Health, Rural Health, and Tribal Health initiatives.

But we have Pay section separate; we might not need to repeat compensation in About the role. However we must preserve all substantive information; we can keep it in About the role but also have Pay section. Duplication is okay? The instruction: "Preserve ALL substantive information; do NOT invent or summarize away detail." Duplication not prohibited but could be redundant. Better to keep compensation only in Pay section, and not duplicate. So we should remove compensation from About the role. Also remove Work Location & Expectations and Professional Development maybe to other sections? Work location could be part of About the role or maybe not needed. Professional Development maybe not a standard heading; could be omitted? But it's substantive. Could be included in About the role as a sentence. However we have no heading for Professional Development; we could include it in About the role as part of description. That's okay. Thus About the role will contain mission, purpose, highlights (excluding compensation). We'll keep work location and professional development as part of About the role. Now Responsibilities: list of key responsibilities. We'll create
  • for each responsibility. Responsibilities list from posting: - Conduct high-volume outbound telephone outreach to prospective CalAIM members identified through health plan referrals, community referrals, and internal lead lists. - Explain WEA services, Enhanced Care Management (ECM), and Community Supports programs to eligible individuals. - Build rapport and engage individuals who may be hesitant or difficult to reach. - Screen potential participants for program eligibility and interest. - Schedule enrollment appointments and coordinate warm handoffs to Care Managers and other clinical team members. - Document all outreach activities, patient interactions, and enrollment outcomes accurately in designated systems. - Maintain productivity standards related to outreach volume, contact rates, and enrollment goals. - Follow established workflows, scripts, and compliance requirements regarding patient communication. - Collaborate with supervisors and program staff to improve outreach effectiveness and patient engagement. - Maintain confidentiality and comply with HIPAA and all applicable privacy regulations. - Participate in team meetings, training sessions, and quality improvement initiatives. - Meet daily outreach targets, including outbound call volume, successful contacts, and enrollment conversion goals. - Maintain accurate tracking of outreach outcomes and disposition codes. - Complete follow-up activities within established turnaround times. - Perform other duties as assigned. Now Requirements: maybe education and experience? The posting has "Qualifications and Education Requirements" and also "Essential Skills And Qualifications". We'll separate: Requirements for mandatory qualifications (education, experience). Qualifications maybe for essential skills? But we have Skills heading allowed. Let's map: - Requirements: Education (High school diploma or equivalent) and Experience (minimum one year of patient centered care experience...). Also maybe ability to work independently, proficiency with MS Office, etc. Those are more skills though. Better: Use Requirements for mandatory qualifications (education, experience). Use Skills for both essential and preferred skills (including knowledge, abilities, etc.). Use Qualifications maybe not needed. But we have headings for Qualifications and Skills; we could use Qualifications for mandatory qualifications and Skills for skills. However the instruction says use those headings if content present. We can decide to use both. Let's assign:

    Requirements

    High school diploma or equivalent.
Minimum of one (1) year of patient centered care experience in patient services, healthcare outreach, call center operations, care coordination, or a related field.

    That's the core mandatory.

    Qualifications

Excellent verbal communication and active listening skills.
    Strong organizational and documentation skills.
Ability to work independently in a remote environment.
Proficiency with Microsoft Office and web-based applications.
Ability to manage a high volume of outbound calls while maintaining professionalism and empathy.

That captures the rest of "Qualifications and Education Requirements" beyond education/experience. Now Skills: include Knowledge, Skills, And Abilities, Essential Skills And Qualifications, Preferred Skills. We'll combine:
    • Strong interpersonal and relationship-building skills.
    • Ability to engage individuals from diverse backgrounds with cultural sensitivity.
    • Demonstrated ability to meet performance goals and productivity metrics.
    • Strong problem-solving and critical-thinking abilities.
    • Ability to maintain professionalism when interacting with vulnerable populations.
    • Commitment to WEA's mission of advancing health equity and improving access to care.
    • Strong interpersonal and relationship-building skills (already). maybe duplicate; we can keep unique.
    • But we need to avoid duplication. Let's list unique items: From "Essential Skills And Qualifications": Actually that section includes compliance & documentation standards etc. Let's parse: "Essential Skills And Qualifications Compliance & Documentation Standards Accurately document all patient interactions in real time using approved systems and workflows. Adhere to all documentation, privacy, and regulatory requirements including HIPAA, CalAIM, and health plan contractual standards. Participate in routine quality assurance reviews and coaching sessions. Knowledge, Skills, And Abilities Strong interpersonal and relationship-building skills. Ability to engage individuals from diverse backgrounds with cultural sensitivity. Demonstrated ability to meet performance goals and productivity metrics. Strong problem-solving and critical-thinking abilities. Ability to maintain professionalism when interacting with vulnerable populations. Commitment to WEA's mission of advancing health equity and improving access to care." So we have compliance/documentation standards as part of essential skills. We'll include those. Then Preferred Skills: - Experience working with Medicaid/Medi-Cal populations. - Experience with CalAIM, Enhanced Care Management (ECM), Community Supports, or population health programs. - Bilingual English/Spanish strongly preferred. - Experience using electronic health records (EHRs), CRM systems, or care management platforms. We'll include those. Thus Skills section will have a list combining all these. Now Pay section: compensation range.

      Pay

The compensation range for this role is $25 - $28 per hour, with final compensation determined based on experience, qualifications, and role scope.

Schedule: not provided; we omit. Benefits: not provided; omit. Now Working Conditions and Physical Requirements: not covered by allowed headings; we could incorporate into About the role or Requirements? Might be considered part of About the role (work environment). We could add a paragraph under About the role about working conditions: "Fully remote position. Requires extended periods of telephone communication and computer use. Must maintain a private, HIPAA-compliant workspace with reliable internet access. Occasional attendance at virtual meetings and trainings required." Also physical requirements: "Prolonged periods of sitting and working at a computer. Ability to communicate clearly via telephone and video conferencing platforms. Ability to perform repetitive hand and wrist movements associated with computer use." We can add that to About the role after mission description. Thus About the role will include mission, purpose, highlights (excluding compensation), working conditions, physical requirements. Now ensure we don't repeat compensation. Let's craft final HTML. We need to output only HTML fragment, no markdown. We'll produce:
