About the role

We seek a compassionate and organized Patient Care Coordinator to serve as a vital link between patients, healthcare providers, community resources, and clinic staff. This position works under the direction of licensed healthcare providers.

Responsibilities

  • Serve as a vital link between patients, healthcare providers, community resources, and clinic staff.
  • Aid with patient intake and basic clinical support services.
  • Coordinate referrals to specialty care and community resources.
  • Aid patients in scheduling follow-up appointments.
  • Aid with prescription facilitation according to pharmaceutical guidelines and policies.
  • Support patient navigation throughout the continuum of care.
  • Complete in-house Medical Assistant training by a licensed health care provider.
  • Escort and room patients.
  • Obtain and document patient vital signs.
  • Communicate pertinent patient information to licensed providers promptly.
  • Maintain patient confidentiality in accordance with HIPAA regulations.
  • Participate in team meetings, case discussions, and interdisciplinary care planning.
  • Support volunteer providers, students, and trainees as appropriate.

Requirements

  • A high school diploma or equivalent is required.
  • One year of experience in healthcare, human services, or customer service is required.
  • A current BLS/CPR certification (or ability to obtain within 90 days of hire) is required.
  • The ability to complete the FHCC Clinical Support Competency Program is required.
  • Strong interpersonal and communication skills are required.
  • The ability to work effectively with diverse and underserved populations is required.
  • Basic computer proficiency is required.
  • Act 33 and 34 Clearances are required.
  • Candidates must not be listed on the U.S. Department of Health and Human Services Office of Inspector General (OIG) List of Excluded Individuals and Entities (LEIE) or any applicable federal or state exclusion database.
