Patient Care Coordinator

About the role

We are seeking a Patient Care Coordinator to perform patient engagement services. You will be the liaison between patients and front desk and Nursing teams to ensure smooth patient flow and a great patient experience.

Responsibilities

  • Greet patients and visitors, check-in patients, ensuring a prompt, positive and welcoming experience in a courteous and helpful manner.
  • Interact with patients and visitors in a professional, courteous, empathetic, and supportive way at all times.
  • Encourage positive interactions and conversations amongst patients and staff.
  • Transport clients to destination as needed.
  • Obtain accurate and updated patient information, such as name, address, insurance information.
  • Perform insurance verification on the date of service.
  • Assist with setting up Helow app on patient’s phones, Health Insurance issues/changing PCP, using Kiosk.
  • Facilitate and resolve patient questions regarding telehealth appointment links.
  • Schedule appointments and manage the patient appointment calendar as assigned.
  • Collect and verify patient information, including medical history and insurance details.
  • Assist in the de-escalation of behaviorally compromised patients. Escalate the matter to management if unable to de-escalate.
  • Perform other duties as assigned by supervisor.
  • Act as a role model for clients and peers.

Requirements

  • High school diploma or equivalent.
  • One (1) year of related work experience.
  • Previous experience in an office or medical administrative support role is required.
  • Previous experience with insurance verification and follow-up is required.
  • Familiarity with medical terminology and procedures is a plus.
  • Knowledge of medical office operations and procedures is beneficial.

Pay

Hourly Rate: $17-20, commensurate with experience and qualifications.
