Patient Care Coordinator

At All American Home Care, the Patient Care Coordinator ensures clients receive consistent, reliable home care services while maintaining compliance and operational accuracy. This role serves as the central point of coordination between clients and caregivers. You help ensure schedules are covered, documentation is accurate, supporting daily scheduling, coordination, and communication. What You’ll Do Coordinate and maintain caregiver schedules to ensure full client coverageCommunicate daily with clients, families, caregivers, and other departmentsManage call-outs and last-minute coverage changes with urgencyMonitor open cases and assist with staffing adjustmentsUtilize HHAeXchange (HHAx) to manage schedules and visit recordsMonitor EVV (Electronic Visit Verification) complianceCorrect missed punches and address visit verification issues in a timely mannerMaintain accurate documentation in internal systemsEnsure professionalism, confidentiality, and compliance at all times What Success Looks Like Schedules are covered and organizedCall-outs are resolved quickly and professionallyCommunication is clear and proactiveDocumentation is accurate and up to dateEVV records are accurate and compliantClients and caregivers feel supportedSchedules are covered consistentlyCommunication is proactive and clear What You Bring Experience in home care, healthcare coordination, scheduling, or case management (preferred)Hands-on experience with HHAeXchange (HHAx)Strong understanding of EVV requirements and visit verification processesStrong communication and problem-solving skillsAbility to stay organized in a fast-paced environmentComfort using scheduling systems, spreadsheets and internal platformsBilingual English/Spanish (a plus)Reliable, detail-oriented, and team-oriented mindset Why Join All American Home Care Supportive team environmentOpportunity for growth within healthcare operationsMeaningful work that directly impacts patient care
