Patient Access Representative
About the role
By The Bay Health, established in 1975, sets the standard for hospice care in the U.S. by focusing on patient-centered decision-making. We offer a range of home-based services including Skilled Home Health Care, Palliative Care, Adult Hospice Care, and Pediatric Care. Our team approach aims to address the practical, social, emotional, and spiritual needs of patients, caregivers, and families.
Responsibilities
- Triage all incoming calls from prospective and current patients, caregivers, members of the medical community, staff, and public, directing calls to the appropriate staff member promptly and courteously.
- Daily audit chart audits by running and reviewing Epic, Excel, and PCSS Admit reports for admission to discharge patients to ensure accuracy, collaborating with internal departments, individual staff members, and external medical community and vendors.
- Collect and distribute interoffice mail.
- Affiliate outgoing mail using the USPS mailing system.
- Assist the Patient Care Support Services Manager on special projects as requested.
Requirements
- High school diploma or equivalent; minimum of one year’s experience performing clerical/administrative tasks, preferably in a healthcare setting.

Pay
$25.00 - $26.00 hourly
Schedule
Monday - Friday, 8:30am - 5:00pm; includes weekend and holiday rotations
Location
On-site; Sonoma with travel to Larkspur office as needed
