Partial Hospital Program (PHP) Case Manager - Per Diem

Pembroke Hospital is a 120-bed acute care, inpatient behavioral health facility located south of Boston, in Pembroke, MA. Situated on 26 acres, we offer inpatient and partial hospitalization treatment to teens, adults and older adults, and our services are designed to assess, stabilize, and treat patients by addressing their primary symptoms and problems. At Pembroke Hospital, all positions begin and end with caring for the patient.

About the role

This per diem Case Manager position is a vital part of Pembroke Hospital’s multidisciplinary treatment team. Program hours are Monday - Friday, 8:00am - 4:30pm. We are looking for a Case Manager who is available to work between 1-3 shifts (8am - 4:30pm), Monday - Friday.

Responsibilities

• Conduct biopsychosocial assessments
• Facilitate groups (strong emphasis on DBT/CBT skills)
• Conduct family therapy sessions
• Document clients’ treatment
• Complete diagnostic assessments
• Provide crisis interventions

Requirements

• Master’s Degree in counseling, social work or a related field
• LCSW, LICSW, LMFT or LMHC preferred
• Ability to multitask and shift responsibilities in a fast-paced environment
• Strong interpersonal and organization skills
• Prior behavioral health experience preferred
