Partial Hospital Program (PHP) Case Manager - Per Diem

About the role

The successful candidate will be responsible for providing comprehensive patient care, managing a caseload, and collaborating with interdisciplinary teams.

Responsibilities

• Assessing patient needs and developing individualized treatment plans
• Conducting physical exams and ordering diagnostic tests
• Maintaining accurate medical records and communicating with patients and families
• Collaborating with other healthcare professionals to ensure coordinated care

Requirements

• Bachelor's degree in Nursing or related field
• Current license to practice nursing in the state
• At least 2 years of clinical nursing experience
