About the role

The Case Manager is responsible for coordinating patient services through an interdisciplinary process, providing a clinical and psychosocial approach across the continuum of care. They assess patients to determine the appropriateness of admission, hospitalization, and care levels, initiate and ensure the completion of necessary paperwork, and facilitate the completion of orders for timely transfers to the next level of care.

Responsibilities

  • Educate patients and families about post-hospital service options and develop appropriate discharge plans.
  • Initiate and ensure the completion of all necessary paperwork.
  • Facilitate the completion of orders as required prior to transfer to the next level of care.
  • Monitor and report quality and risk management issues as appropriate.
  • Communicate with the department director, LOS, and financial information as needed.
  • Communicate with physicians and other healthcare professionals to support the discharge plan.
  • Coordinate patient care processes to achieve desired quality outcomes and identify inappropriate resource utilization.
  • Facilitate patient and family education and promote continuity of care.
  • Communicate with the multi-disciplinary team and participate in care conferences.
  • Review the patient plan of care with the multi-disciplinary team.
  • Communicate in the medical record and verbally with the team to coordinate interventions and facilitate continuity of care.
  • Communicate daily with patient care staff to provide continuous assessment, evaluation, and continuum planning.
  • Ensure patient rights are offered when appropriate.

Requirements

  • Registered Nurse (RN) license in Colorado or a Master of Social Work (MSW) or Licensed Clinical Social Worker (LCSW) license in Colorado.
  • At least 2 years of experience working as a clinician in an acute care setting.
  • Live in the Boulder CO regional area.
  • Willing and able to work one weekend shift every two weeks and availability as needed.

Qualifications

  • Experience or certification in case management.
  • Knowledge or understanding of community resources, policies, and procedures.
  • Solid clinical acumen with the ability to use sound judgment, deductive reasoning, and problem-solving skills.
  • Solid written and verbal communication skills.
  • Ability to learn and understand various clinical software applications.
  • Proficient typing skills.
  • Ability to solve problems and actively pursue resolution.
  • Organizational skills and ability to stay focused on details.
  • Collaborative skills and ability to interact with people of diverse backgrounds.
  • Self-motivated and able to function in a fast-paced work environment.
  • Maintains quality and productivity standards.

Skills

  • Understanding of managed care and Medicare health plans.
  • Reimbursement related to post-acute services within the continuum of care.
  • Effective communication and problem-solving skills.

Pay

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401k contribution (all benefits are subject to eligibility requirements).
