About the role
This role involves providing individual and group social work services to patients and their families across various healthcare settings.
Responsibilities
- Conducting psychosocial assessments
- Delivering culturally competent care using diverse modalities
- Advocating for patients to ensure access to healthcare
- Collaborating with interdisciplinary teams, sharing assessment findings and recommending care adjustments
- Offering psychoeducation, counseling, and crisis intervention
- Formulating and implementing safe, timely treatment and discharge plans
- Coordinating with post-acute providers
- Utilizing community resources
- Maintaining accurate documentation
- Representing the department in committees
- Supervising social work students
Qualifications
- Master of Social Work
- Current license as a Master or Clinical Social Worker in the relevant state
- 1–3 years of relevant experience
