Part-Time Social Worker

About the role

Provide psychosocial support and social work services to hospice patients and their families in various settings, including inpatient units, nursing homes, and home care.

Responsibilities

• Develop and implement social work care plans based on patient and family needs, setting goals and making recommendations.
• Offer pre- and post-bereavement services to support grieving families.
• Serve as a member of the hospice interdisciplinary team and act as a liaison with inpatient and nursing home staff.
• Maintain accurate documentation of assessments and interventions.
• Participate in discharge planning when appropriate.

Qualifications

• Graduate from an accredited School of Social Work.
• Minimum of one year of healthcare social work experience.
• Knowledge of Word and Excel preferred.
• Valid New York State MSW license required.
• NY State Driver’s license preferred.
• Strong organizational, communication, and listening skills.
• Ability to work collaboratively within an interdisciplinary team and proficiently input clinical data.
