About the Role

Support patients in home health, hospice, and personal care services by conducting psychosocial assessments, providing counseling, and coordinating resources to enhance independence and quality of life.

Responsibilities

• Conduct psychosocial assessments and develop personalized care plans.
• Provide counseling, crisis intervention, and resource coordination for patients and families.
• Collaborate with an interdisciplinary team to advocate for patient needs.
• Connect families with community resources to enhance independence and quality of life.
• Support patient well-being, foster community connections, and promote quality care within a flexible and supportive environment.

Requirements

• MSW degree required.
• Compassionate, patient-focused approach.
• Strong communication and interpersonal skills.
• Ability to work independently in patients’ homes.
• Experience in psychosocial assessment and crisis intervention preferred.
